At its core, is a story about the dynamics of unrequited love. The narrative follows Ayan Sanger (Ranbir Kapoor), a wealthy, aimless musician who aspires to be a singer but lacks the life experience to give his art soul. His life changes when he meets Alizeh Khan (Anushka Sharma), a spirited young woman who is dealing with her own romantic baggage involving her ex-boyfriend, Ali (Fawad Khan).
When Alizeh marries Ali, a devastated Ayan finds temporary solace in a brief, poetic relationship with a sophisticated older woman named Saba (). Saba eventually helps Ayan realize that his heart still belongs to Alizeh. The film's final act takes a dramatic turn when Alizeh is diagnosed with stage 4 cancer, forcing both characters to reconcile their definitions of love and companionship before her eventual death.
