In simpler terms: The Engine Control Unit (ECU) has detected that Cylinder 4 is not combusting correctly. To compensate, the ECU tried to “learn” and adjust the injector’s fuel delivery timing and quantity. It hit the maximum safety limit (high rail pressure), gave up, and threw the P306D code.
