Deep in the Agnod Dungeon, behind the third lava fall, is a scrying pool. It is a secret frequently missed due to the lava damage. If you wash your face in the pool (activate it), instead of seeing your player character, you see a monitor displaying the real world behind the game.

Near the Ark Southern Quarter’s well, there is a locked trapdoor with a level 100 lock (Master). Inside: You will find a horrifying tableau of mannequins posed in ritualistic positions, bloodstained letters from a noble patron, and a unique dagger called "The Finisher." The letters reveal that the killer was never caught because they work for a faction within the Order involved in human transmutation experiments. This quest is never given to you; you must discover the key on the corpse of a beggar near the Harbor.
